The concept

Hansei is the practice of honestly acknowledging what didn’t work — not to blame, but to learn. Even in success, one asks what could have been better.

In engineering

Every incident becomes learning. We run post-mortems with no witch hunt: what failed, why, and what changes so it doesn’t repeat. An error recorded and understood is an error that doesn’t return.
